Career Day Permission Form - Please return page 2 to the Guidance Office for the November field trip.

Campus Visit Score Card - helpful tool used to rate and organize your college visits.

College Credit Plus Schedule Submission - After you schedule classes at the university, you must complete this form and attach your college schedule so your Copley schedule can be updated. (Also review this form to turn in your book list)

College Credit Plus Release Form - CCP students should complete if they must leave Copley HS to attend their CCP class.

Compact Withdrawal Form - Must be completed and signed to withdraw from a Compact Program.

Credit Flex Application - Students can apply to receive credit for a new course or test out of a current course. (Deadline for Semester 1 April 30th, Deadline for Semester 2 November 30th)

Early Graduation Form - Must be completed and turned in before October 1st.

4 Year Plan 2026 and Beyond - Use this form to plan your courses while at CHS.  

Level Change Form - If a teacher recommends a level change, students must complete this form. (Students can't change levels on their own)

Pre-Planned Absence Form - Students complete this form(in advance) if they will need to miss school in the future.  

PSAT Registration Form- Sign up to take the PSAT.

PSAT Practice for Students - review for the PSAT and learn about the test.

Schedule Change/Drop Form- Students can drop a course the first 20 days of each semester ONLY if they have a full schedule.

Transcript Request Form - Return for CCP Application (IF APPLYING TO COLLEGE PLEASE REQUEST IN NAVIANCE)

Transcript Release Form - Sign and return to the guidance office to release transcripts to schools for the purpose of college application, scholarship, NCAA.

Volunteer Hours Confirmation Letter - Students must complete a total of 12 hours of community service from two different volunteer experiences no later than the last Friday in April of their senior year. The hours must be completed during their junior and senior years.
